REGISTRATION FORM

NEW STUDENT FAMILY FORM

To ENROLL OFF-LINE, follow the steps below: 
  1. Download and print the required forms.
  2. Complete information requested on each form.
  3. Be sure to calculate the full semester payment for all classes. If applicable, include 2x tuition listed to make up the entire 18 week session.
  4. The second installment for those paying by credit card will be charged mid-semesterA registration fee of $30/family will be charged for new family set up in the first installment. 
  5. Families with more than one child attending ChinaKids are entitled to a $30 credit for EACH ADDITIONAL child to be applied to the current tuition payment. 
  6. Submit your payment in one of three ways:
    • Credit Card - follow instructions on the form.  Be sure to complete billing address and verification code data requests.
    • Check - mail in to the address listed above
    • Online via Paypal
  7. Submit enrollment form in one of two ways:
